In addition, we request that you enter your name, contact telephone number, dates of birth for you and your spouse if you have one, and your employee status. This data is necessary for the Premium Calculator to develop EZ Pick plan design options that are specifically developed based on your resident state to assist you with your final coverage elections.

If you decide to apply for long-term care insurance through TBG we will request additional personal information that is required to complete the application process.
